1. Download and complete claim form(s)2. Prepare all the supporting document(s)3. Submit completed claim form(s) with all supporting document(s) within 90 days of the consultation/treatment date4. Once all required documents are received, your claim will be processed within 7 working days5. Settlement letter and/or email will be sent to you once your claim is approved

<ul><li>Copy of reports for histopathology/endoscopic/diagnostic/laboratory tests</li><li>Copy of operating theatre summary report(s)</li><li>Referral letters from any specialist doctors</li></ul>
